If you are interested, check off the type of sponsorship you are interested in, provide your Business, Name, Location of sponsorship and give this form to a JBN Representative (Rabbi Minkowicz, Shawn Seliger or Maralee) for review by the Hosting committee. We will get back with you within a few days to discuss available dates and details.
